Permission to deposit ALISE 2005 Conference Presentation in DLIST: This is a single page PDF file. If you want your ALISE 2005 conference presentation made available through DLIST, please print a copy of this form, complete, sign it and send it to DLIST. For works that are by more than one author, every author must sign a permission form.
Permission to Deposit in DLIST: This is a 2-page PDF file of a generic permission to deposit in DLIST. If you, an author, wish to avail yourself of DLIST services (see above), print a copy of this form, complete and sign it. Send it to DLIST. Again, for works by multiple authors permissions must be signed by every author who is listed on the work.
